The Venus Free Whitening Offer is limited to 1 (one) Patient Kit of Venus White and 1 (one) set of custom made whitening trays, valued at up to $400. Eligibility is determined by a dental professional, and requires that teeth be in good condition. This is determined by a new patient examination and cleaning (D0150 comprehensive oral examination) that may include any of the following services: bite wing x-ray (D0210 Intraoral-complete series [including bitewings]), panoramic X-ray (D0330 Panoramic film), and oral teeth cleaning (D1110 Prophylaxis-adult).

Any such services deemed medically necessary by a dental professional are not included in the Venus Free Whitening Offer. These additional services may not be covered by your dental insurance and a fee may be assessed by the Dental Professional. Be sure to discuss any charges with your dental professional and your insurance company before starting treatment.

The patient has the right to refuse any other dental procedures not associated with the Venus Free Whitening Offer.

The Dental Professional has the right to determine the Free Whitening as detrimental to the patient's oral health and may therefore withdraw the Free Whitening offer until the patient's oral health is deemed medically healthy to receive Whitening. The Dental Professional has the right to charge a standard cancellation fee for appointments cancelled within 24 hours of the appointment time or for patients who do not show up at the appointed time to the office.
